Warts

Warts – the disease of the virus nature which is characterized by emergence on integuments of small roundish speakers of outgrowths. Distinguish several types of warts: ordinary - are localized on fingers of hands, a face, a hairy part of the head; plantar, flat (youthful) - settle down on a face, the back of brushes, peaked (condylomas) – arise in anogenitalny area more often. Warts are inclined to distribution, a recurrence and rapid growth; when traumatizing bleed, can change a form, color, to become painful; at self-treatment cause emergence of hems and scars, malignant regeneration.

Warts are inclined to distribution, a recurrence and rapid growth; when traumatizing bleed, can change a form, color, to become painful; at self-treatment cause emergence of hems and scars, malignant regeneration.

    Etiology and pathogenesis

The Virus of Papilloma of the Person (VPP) causing formation of warts is widespread in all age groups and most often simple, plantar and flat warts meet. Not only skin, but also mucous membranes of an oral cavity, a nose, a bladder, vocal chords, etc. is subject to defeat of VPCh.

Infection with the VPCh virus results from contact with the infected or sick person, and also at contact with sick animals. Cases of infection with a household way, through things, toys and public objects are frequent. After penetration into a human body the virus begins to breed actively in skin blankets.

Autoinokulyation (self-infection) meets quite often; warts in okolonogtevy area are connected with a habit to gnaw nails or to bite fingers, and emergence of flat warts in the person is connected with shaving, a peeling and other cosmetic procedures as a result of which damage of integuments is possible.

The virus of papilloma gets into a human body through skin microinjuries; the people who are actively visiting pools, gyms of a bath and saunas are most subject to infection. Also employees of poultry farms and occupied with cutting of meat or fish get into risk group, their warts affect hands and forearms.

The incubatory period usually from one and a half to five months, but even at infection of external displays of a virus can not be. Simple warts, youthful (flat) and plantar are most widespread; peaked warts are called still condylomas.

Simple (vulgar) warts. Simple warts are localized on palms, a back part of brushes and on fingers, seldom develop on a face and are even more rare on mucous membranes; externally look as roundish dense small knots, coloring of skin is not changed, but there can be pink or yellow shades. Such warts have multiple character and have tendency to merge. Affected areas of skin to the touch rough and uneven. Among the lump of warts one usually the largest, it still call maternal after which removal by the rest often disappear independently.

Vulgar warts make 70% of all skin warts; are most widespread among children and school students.

() Plantar warts. Are localized on soles and cause painful feelings when walking, externally they are similar to ordinary warts.

Palmar and plantar warts are widespread among young people and people of advanced age. These warts look as dense educations with a reinforced horn layer of skin. They need to be differentiated from usual attritions of skin and callosities, and also to carry out the differential diagnosis with a plantar papule at syphilis. Inconvenient footwear and the increased perspiration promotes spread of an infection.

Okolonogtevy warts are option of ordinary warts, most often occur at the children gnawing nails and skin around them. After their removal a recurrence happens quite often.

Flat or juvenile warts meet in 4% of cases, affect skin of hands, faces, mucous. Flat warts can be localized on a penis head, a neck of a uterus and on mucous a rectum. The age category from 10 to 25 years gets into risk group.

Threadlike warts settle down on a neck, skin a century, in axillary hollows, on mammary glands and in inguinal area. Externally look as soft papules, sometimes on a leg that often conducts to their travmatization. Color of threadlike warts varies from corporal to dark brown.

The virus nature of threadlike warts is confirmed by their tendency to an autoinokulyation, and hormonal conditionality - communication with diabetes, approach of a menopause and with other states at which the hormonal shift is observed.

Peaked warts (condylomas) because of a lobular structure and a pasty consistence are externally similar to a cauliflower or a cock crest. Color of warts pink or corporal, however, at friction they become bright red, and when traumatizing easily bleed. Have a narrow leg and are inclined to formation of extensive conglomerates. Are localized on genitals and in a crotch, at children nasolabial folds are surprised more often.

    Diagnostics

The diagnosis is made on the basis of external signs. But warts need to be differentiated with red flat it is deprived of which wax-like gloss and violet-red color of papules is characteristic. Differ in lack of inflammatory infiltrate and a red-violet nimbus on the periphery from warty tuberculosis of skin of a wart.

During removal of the central part of a plantar wart the gentle sosochkovy layer is bared, and on the periphery there is a dense horn ring. At capture of a biopsy such picture is confirmation of correctness of the diagnosis.

    Treatment of warts

The main method of treatment consists at a distance or destruction of warts by means of medicines or mechanically. Antiviral ointments interfere with spread of an infection on not affected areas of skin. Appoint oksolinovy, tebrofenovy and other ointments in the necessary percentage concentration.

Cryodestruction and electrothermic coagulation render good therapeutic effect at most of patients. Laser removal depending on type of a beam gives effect of evaporation or coagulation of the affected skin. After local anesthesia make layer-by-layer removal of a wart, depth of penetration and time of influence depend on the size and localization of education. Such type of treatment practically does not leave hems, and pigmentation of skin does not change. On the place of a wart there is a deepening which drags on in 10-14 days.

Electrothermic coagulation is carried out under local anesthesia too, the principle of action consists in ability of current of high frequency to coagulate fabrics, the metal loop just cuts off a wart, and short-term influence of high temperature prevents bleeding and spread of an infection in the hematogenic way. On the place of a wart there is a small crust which disappears in a week. If the wart was the big size, then there can be a hardly noticeable scar.

Surgical treatment is carried out if warts struck the extensive site of fabric. Excision of fabrics is carried out under local anesthesia, with imposing of intracutaneous cosmetic seams which in 7-10 days remove; after treatment there is a light imperceptible scar. Cryodestruction by liquid nitrogen by the principle of action is similar to electrothermic coagulation, only death of fabrics happens not because of heating, and owing to deep freezing.

Depending on a course of disease, it is possible to combine mechanical and medicamentous therapy. The histologic research of remote material is obligatory. Efficiency of treatment fluctuates from 50 to 94%, a recurrence happens at a quarter of patients. What began treatment earlier, that it is more effective, it is also expedient to carry out the general antiviral and antirecurrent therapy. The forecast and probability of emergence of a recurrence depends on timeliness of treatment and for a condition of immune system.

Warts - treatment should be carried out only under the supervision of a doctor. Self-treatment is unacceptable!!!
